Enhanced Care Management (ECM)

Our ECM team provides high-touch care management services for individuals with complex health and social needs through: Personalized Care Plans – Addressing individual needs holistically. Comprehensive Coordination – Connecting medical, behavioral, and social services. Social Determinants of Health Support – Assisting with food, housing, and transportation. Regular Check-ins – Ensuring ongoing support and improvement in well-being.

Who Qualifies for CHW & ECM Services?

These services are available to Medi-Cal members who experience complex health and social needs, including: Individuals experiencing homelessness Adults & children with serious mental illness or substance use disorders High utilizers of emergency services People with complex chronic conditions Individuals transitioning from incarceration or institutional settings

No Wrong Door Access

At Common Good Community, we believe that crises, emergencies, and urgent needs don’t follow a 9-5 schedule—and neither do we. That’s why we provide 'No Wrong Door' access to support individuals whenever they need help.

Through our Coordinated Care Companion app—Coming Soon, individuals can:

  • Access resource directories like 211 and Find Help for essential services.

  • Call a Coordinated Care Companion directly for immediate support.

  • Submit an Assistance Request or Appointment Form to initiate an intake whenever a crisis or urgent need arises

  • Find additional resources tailored to support their unique needs.
