Hi, I'm Chi Qian Chen, the current Community Health Work Program Manager with Common Good Coordinated Care.

I manage clinic operations, connect clients to resources addressing social determinants of health, and coordinate preventive health screenings for those at risk of chronic diseases. I'm passionate about public health and earned my Community Health Worker Certification at Valley High Health TECH Academy.

With a Biochemistry and Molecular Biology degree from UC Davis and experience in community health, I aim to serve medically underserved communities through a medical career. I've completed three internships in Federally Qualified Health Centers and have also worked as the Associate Program Director for Sowing Seeds Health. Additionally, I was a Community Health instructor at the Elk Grove Adult and Community Education Center.

My vision is to promote healthier lifestyles in underserved communities and empower clients to achieve self-sufficiency so they can maintain their best health.
