A Companion at Your Side…

Common Good Coordinated Care Companions: Providing Hope and Support Through Our Engage, Educate, Equip, and Empower Model

September is Suicide Prevention Month—a time to shine a light on the mental health struggles many face and offer pathways to hope. At Common Good Community, we are committed to being there for those who feel overwhelmed, hopeless, and burdened by life’s obstacles. Through our dedicated Coordinated Care Companions (CHWs) and the power of Motivational Interviewing, we help individuals navigate their challenges with compassion and care.

Our approach is built on four pillars: Engage, Educate, Equip, and Empower. Here's how we integrate these pillars into our daily work, ensuring 24/7 access to help and support—because crisis doesn’t follow a 9-to-5 schedule.

Engage: Building Trust and Connection

The first step in our approach is engagement. We actively connect with individuals through phone, text, email, and even QR codes that make accessing support as easy as a quick scan. We understand that during moments of crisis, a friendly, understanding voice on the other end of the line can make all the difference.

  • 24/7 Access: Whether it's 3 a.m. or during a busy afternoon, our team is always ready to listen. We provide round-the-clock access via phone, text, email, and our user-friendly app that allows members to reach out for help or schedule appointments anytime—no waiting, no unanswered calls.

  • Motivational Interviewing: Our CGCs use this compassionate, non-judgmental approach to build rapport and trust. By truly listening and reflecting on what individuals share, we create a safe space where people feel valued and understood.

Educate: Providing Knowledge and Understanding

Education is empowerment. We believe in equipping individuals with the information they need to better understand their mental health and the steps they can take towards improvement. Our CGCs provide tailored health education that addresses each person’s unique needs.

  • Personalized Guidance: Our CGCs take the time to explain health conditions, coping strategies, and available resources in ways that make sense for each individual. This personalized education helps demystify the complex world of mental health and makes the path forward clearer.

  • Resource Sharing: Through our app, individuals can access a library of educational materials, videos, and self-help tools. These resources are available 24/7, so help is never out of reach.

Equip: Offering Tools and Resources for Immediate Support

When someone is facing a crisis, they need practical support—fast. We equip individuals with the tools and resources they need to manage their health challenges right now, reducing the overwhelming feeling of being stuck.

  • Immediate Access to Services: From health screenings to referrals, our CGCs help coordinate care without delays. Individuals can sign up for appointments directly through our app, ensuring they can access the services they need, whenever they need them.

  • Digital Connections: With QR codes and direct links, accessing support has never been easier. Individuals can connect with CGCs, access mental health resources, or schedule follow-ups with a simple click or scan.

Empower: Helping Individuals Regain Control of Their Lives

Our ultimate goal is to empower individuals to take charge of their mental health and well-being. Through ongoing support and encouragement, we help people rediscover their strengths, set achievable goals, and take steps toward a more hopeful future.

  • Goal Setting and Follow-Up: Our CGCs work with individuals to set small, manageable goals that build momentum over time. Regular check-ins provide a sense of accountability and continued support, keeping people on track even when challenges arise.

  • App-Based Support: Our app not only allows for scheduling but also tracks progress, sends reminders, and provides motivational content to keep individuals engaged. Empowerment is about feeling capable, and our tools are designed to foster that sense of control and confidence.

Crisis Doesn’t Keep Office Hours—And Neither Do We

At Common Good Community, we know that life’s most difficult moments often happen outside of office hours. That’s why our services are designed to be accessible 24/7—whether through phone, text, email, or our app. We reject the idea that help should only be available from 9 to 5, and we are committed to ensuring that every call, message, or appointment request is met with a timely, compassionate response.

If you or someone you know is struggling, please reach out. Our Coordinated Care Companions are here to engage, educate, equip, and empower you every step of the way. You are not alone, and together, we can navigate these challenges, find hope, and build a brighter future.

If you are in crisis, please contact the Suicide & Crisis Lifeline at 988 for immediate support. Your life matters, and help is always available.
